フランス語 questionnaire から。ラテン語 quaerere（尋ねる）が語根で、quest（探求）+ -ion（名詞化）+ -naire（～に関するもの）と分解できる。関連語: question（質問）、query（問い合わせ）。「尋ね求める」が原義で、体系的に問いを並べた用紙という意味に発展した。
Questionnaire was borrowed from French questionnaire, a noun formed from question with the suffix -naire. Question ultimately comes through Old French from Latin quaestio, “an inquiry” or “a seeking,” related to quaerere, “to seek or ask.” The English word became established for a structured list of questions used to gather information. Although its spelling can be analyzed through question and a French suffix, it should not be treated as a native English combination of quest, -ion, and -naire.
