referral は refer からできた語で、ラテン語 referre「後ろへ運ぶ」に由来します。re- は「後ろへ」、fer は「運ぶ」、-al は名詞化で、transfer や offer も同じ ferre 系です。相手を別の人へ「運んで回す」感覚から、紹介や照会の意味になりました。
